Your gift will help fund research and public and professional education programs.

I would like to help support the ongoing activities of MODEP through:

	Corporate Sponsorship MODEP is seeking Corporate sponsorship to enable the program to continue its efforts to educate minority communities about the need for organ donation.
	Individual Contribution MODEP accepts individual contributions, which can be given "in memory" of a loved one, "in honor" of an individual or for a "special occasion".
	Organizational / Group Contribution MODEP accepts contributions from organizations and groups interested in supporting its mission.
	In-Kind Contribution MODEP accepts in-kind contributions of supplies and/or services such as office supplies, printing services, promotional items, etc. Such contributions help us allocate a greater percentage of our resources directly toward our mission and are greatly appreciated.
	Volunteering My Time MODEP welcomes the assistance of committed individuals interested in volunteering their time to support our mission.
